For first time patients we ask that you arrive 15 minutes prior to your appointment to fill out some paperwork or you can download our New Patient Form here and fill it out at your leisure. Please bring it with you when you arrive for your appointment.

For our Holistic Wellness Coaching clients please read and sign our Disclaimer Form.

Cancellation

Golden Door Wellness does have a 24 hour cancellation policy. Circumstances are taken into consideration, but we do reserve the right to charge the full fee in the event that appointments were not cancelled 24 hours in advance.

Insurance

Golden Door Wellness does not currently take insurance directly. However, we recommend that you contact your insurance company to see if they cover acupuncture and if so for what specifically. If your insurance company does cover acupuncture, we will be more than happy to provide you with a receipt containing all pertinent information for you to submit for direct reimbursement.

Health Savings and Flexible Spending Accounts cover cost of treatments and we will be more than happy to provide any receipts that you may need for full coverage.

In the event that your insurance does not cover your treatments or you do not have an HSA or FSA,  you can still consider your payments as medical expenses that you may be able to write off on your taxes.

Payment

Payments can be made by Cash, Check, or Credit Card (Including FSA or HSA Credit Card).
